What is Fast Track Continuing Care Assessment and when should it be used?

Answer 1

• Patient has a rapidly deteriorating condition.
• Usually have less than 6 months to live.
• May be entering the terminal phase.
• To enable their needs to be urgently met. (e.g. provide appropriate end of life support)
• NHS continuing care funding needs to urgently be put in place.

Question 2

What is a Health Needs Assessment?

Answer 2

• A tool to assess the needs of a patient by MDT so see if they are eligible for continuing care funding.
• Not urgent as it can take some time.
• Do not use if the patient is rapidly deteriorating.
• Example: Mum in care home - chronic MS.
