Direct smooth muscle relaxation:

  1. VENOdilation of periph (= Inc peripheral venous capacitance)
  2. Dec preload (dec LVEDV+LVEDP. dec LV systolic wall stress + dec myocardial O2 demand - inc exercise tolerance :: relieves angina )
  3. Mod dec in AL, d/t dec SBP
  4. Mild coronary artery dilation

— headache, flushing, hypotension, reflex tachycardia (d/t dec BP : prevent by co-adm beta-blockers)
— rapid tolerance to Nitrates : relieved by nitrate-free pd each night
