Cardiovascular Nursing Flashcards

1
Q

What is Morphine Sulfate used for cardiac drug therapy

Q

What is the therapeutic action of nitroglycerin

A

Reduce pain, decrease preload and afterload, increased myocardial oxygen supply, increase collateral circulation to ischemic areas of myocardium

Q

Why is it important to keep the client sitting position if pulmonary edema is suspected in a client with MI

A

Vessels may become engorged and leak fluid into the lungs

Q

What are the clinical manifestations of acute pulmonary edema

A

Severe dyspnea- rate 30 higher,
Breath sounds- bubbling crackles, wheezing and rhonchi throughout lungs, cough with frothy, blood tinged sputum, tachycardia, cold clammy skin

Q

Why are stool softeners encouraged in a patient that has MI

A

Prevent straining; vagal stimulation causes bradycardia and provokes ventricular arrhythmias and this can cause sudden increase in venous return to the heart which will overload and weakens the heart

Q

What steps do we take in managing pulmonary edema

A
  1. High fowler, legs dangling
  2. VS, PA wedge pressure, I&O hourly
  3. Oxygen
  4. Morphine IV
  5. Diuretics IV (furosemide or bumetanide)
  6. Nitroglycerin IV
  7. Daily weights

Q

Signs of Hypokalemia

A

Weak pulse, faint heart sounds, hypotension, weakness

*diuretics can lead to hypokalemia which markedly weakens myocardial contractions

A

Low cardiac output syndrome- caused by hypovolemic due to loss, hypotension

17
Q

T/F there is a risk for pulmonary embolism occurring after the 3rd post op day

A

True, common in clients with coronary bypass surgery
