Acute MI

A
  • Hyperacute T waves -> elevate ST segments-> T wave inversion -> Q waves. And repeat every 20 mins x 2 hrs if normal at first.
  • The ST elevation needs to be >0.1 mV in >=2 contiguous leads
  • OR a new LBBB (obscures usual ST-segment analysis)

  • II, III, avF: inferior leads, RCA
  • I, avL, V5, V6: lateral, LCX

A
  • Often see sinus brady bc RCA supplies SA node
  • Doesn’t usually require tx unless hypotension.
  • If rate is slow enough to cause dec CO + BP, then give IV atropine.

A

HyperK: peaked T waves, flat P waves, prolonged PR + QRS, sine wave + brady if severe.

HypoK: dec T wave, U wave
