Cardiogenic shock Flashcards
1
Q
Early signs of shock (1)
A
MAP is under 10mm Hg from baseline
Effective compensation
Slight increase in HR
2
Q
Compensatory signs of shock (2)
A
drop in MAP by 10-15mm Hg
Increased
- renin
- ADH
- Vasoconstriction
- HR
- Potassium
Decreased
- Pulse pressure
- pH
Restless and apprehensive
3
Q
Progressive signs of shock (3)
A
Sustained drop in MAP by over 20mm Hg
Tissue and organ hypoxia
Oliguria
Weak, rapid pulse
lower pH
sensorineural cyhanges
4
Q
Refractory signs (4) irreversible
A
Excessive cell and organ damage
multi organ failure
death
5
Q
Clinical manifestations post MI
A
Need O2 admin and pain control (morphine)
Monitor Hemodynamics and labs (troponin)
Not as aggressive w/ fluid therapy
Give meds
- Dobutamine
- Nitroglycerine
- Antiarrhythmics
- Furosemide
- Thrombolytics
