Cardiogenic shock definition
-clinical condition of inadequate tissue (end-organ) perfusion due
to the inability of the heart to pump an adequate amount of blood
Causes of cardiogenic shock
-ACS
-arrythmias
-acute decompensated heart failure
-mechanical causes eg. acute mitral incompetence, cardiac wall rupture etc.
Pathophysiology
-pump failure lead to
1. reduced cardiac output -> reduced tissue perfusion -> reduced coronary perfusion
2. increases heart rate -> increased myocardial O2 demand -> ischaemia and infarction
-activates compensatory systems to increase BP
1. BP drop detected by baro/chemoreceptors which cause vasoconstriction, increase HR, therefore increasing BP
2. Renin angiotensin aldosterone system, more water retained, vasoconstriction, increased BP
Signs and symptoms
-tachycardia
-hypotension
-delayed cap refill
-tachypnoea
-reduced O2 sats
-reduced GCS
-oliguria
-pallor/ clammy
