Q

First line intervention if cardiogenic inadequate perfusion with crackles, or initial fluid bolus insufficient.

A

Adrenaline infusion commencing at 5mcg/min (5mL/hr) titration to SBP 100. (Max 250mcg/min). Consider IV bolus of 10mcg, increase to 50-100mcg IV boluses if syringe pump unavailable. Continue 250mL fluid boluses if chest clear up to 20mL/kg.
