Previous viral infection
pain relieved by sitting forward
WPW syndrome
on ECG:
Delta waves
Short PR interval
Broadened QRS complex
ventricular septal defects
May present with symptoms of heart failure
Loud, harsh pansystolic murmur loudest at lower left sternal angle
stable angina
Chest pain on exertion
Radiates to jaw, neck or arm
Relived by rest or GTN spray
right sided heart strain
PMHx of PE, pulm hypertension, pulm stenosis
Sinus tachycardia on ECG
Right axis deviation
P pulmonale
aortic dissection
Sudden tearing/ripping chest pain
Typically radiates to back
PMHx of connective tissue issues eg. marfan’s syndrome, VHD
aortic regurgitation
High pitched early diastolic murmur
Best heard at 4th ICS left sternal edge
Radiates to carotids
aortic stenosis
Ejection systolic murmur
Best heard at 2nd ICS right sternal edge
Radiates to carotids
mitral regurgitation
Pan-systolic murmur
Best heard at apex
Radiated to axilla
May displace apex
mitral stenosis
Mid-diastolic murmur
Best heard at apex
Tapping apex beat
Radiates to axilla
Elevated JVP, ‘a’ wave on ECG
myocarditis
ECG shows:
sinus tachycardia
non-specific T-wave inversion
ruptured AAA
sudden onset abdominal pain radiating to the back with an expansive abdominal mass
PVD
presents with intermittent claudication
severe PVD
pain in calves even at rest
ulceration and tissue necrosis
diminished or absent peripheral pulses
atrial fibrillation
chest pain, SOB, etc
palpitations
irregularly irregular pulse rate
