Asprin Class
Platelet inhibitor, anti-inflammatory
Aspirin Indications
New onset of chest pain
Aspirin Contraindications
Hypersensitivity, recent bleeding, stomach ulcers
Aspirin Dose
Dose: 160 mg - 324 mg, depending on the amount patient has taken within the past 12 hours
Nitro Class
Vasodilator
Nitro Indications
Acute angina, ischemic chest pain, heart failure, pulmonary edema
Nitro Contraindications
Hypotention, hypovolemia, intracranial bleeding or head injury, pericardial tamponade, severe bradycardia or tachycardia, ED drugs within the last 48-72 hours, inferior wall myocardial infarction.
Nitro Dose
Adult dose: 0.4 mg sublingual or transdermal, every five minutes until pain is relieved (max three doses) Paste ½ inch – ¾ inch (15-30 mg) Transdermal to chest
Mag Sulfate Class
Electrolyte, Smooth muscle relaxant
Mag Sulfate Indications
Torsades de pointes, hypomagnesemia, Pulseless VF /VT that is refractory to amiodarone
Mag Sulfate Contraindications
Heart block, myocardial damage
Mag Sulfate Dose
Adult dose: Polymorphic VT or Torsades de Pointes: 1 -2 G IV/IO over 15-30 minutes
Epinephrine Class
Sympathomimetic, Alpha1, Beta1, & Beta2 Agonist (vasoconstrictor, chronotropic, ionotropic, bronchodilaltor)
Epinephrine Indications
Cardiac arrest (asystole, PEA, VF and Pulseless VT) symptomatic bradycardia as an alternate infusion to Dopamine.
Epinephrine Contraindications
None in the emergency setting.
Epinephrine Dose
Adult dose: 1 mg every 3-5 minutes IV/IO
2-10 mcg/min Conversion infusion or continued refractory hypotension/bradycardia: 2-10 mcg/minute IV/IO.
