Cardiac Implantable Electronic Devices (CIED) Flashcards

1
Q

What is a cardiac implantable electronic device?

A

electronic devices that can sense heart rate and sometimes rhythm and electrically stimulate the heart to beat at a pre-set rate and/ or electrically stimulate the heart to beat in a regular rhythm.

Q

What are 3 different types of CIEDs

A
  1. Pacemakers
  2. Implantable cardioverter defibrillators (ICD)
  3. Cardiac resynchronization theraphy (CRT)

placed when patients have low heart rates or arrhythmias. There are one or two leads, implanted in the right atrium and/ or the right ventricle.

placed for primary or secondary prevention for patients with runs of tachyarrhythmias requiring defibrillation to convert them. ICDs can also be programmed to pace the heart. There are also one or two leads, implanted in the right atrium and/ or right ventricle.

Q

devices are a treatment for heart failure. They have leads placed in the right atrium and right ventricle and have a third lead is placed on the left ventricle via the coronary sinus. The two ventricular leads cause the heart to contract at the same time to improve forward flow.

Q

Indications for a Pacemaker

A

syncope or pre-syncope, dizziness, congestive heart failure, mental confusion, palpitations, shortness of breath, exercise intolerance. There are a variety of conduction issues that cause very slow heart rates, very fast heart rates or a combination of very fast and very slow heart rates.

Q

Indications for an ICD

A

survival of VT/VF arrest, primary prevention in patients at high risk for VT/VF, structural heart disease, EF<30% and some high risk subgroups with genetic electrophysiological disorders.

Q

Indications for a CRT

A

implanted to improve heart failure symptoms. If patients meet the indications for an ICD or may in the future, a CRT-D generator is implanted. If they do not, a CRT-P generator is implanted.
