Found in fox glove
Name: dioxin, digitek, lanoxin
Digoxin 3 main functions

Primary indication for med
Increases myocardial contration
Increases CO
Decreases oxygen use by making each contraction become more efficient

Mainly why the O2 compensation is decreased

Decreased conduction from SA node to AV node
Main indications for cardiac glycosides
CHF (main indication):
-Slows the heart down which decreases O2 consumption
-Increases the force of each contraction
A-fib:
Slow conduction thru SA and AV nodes

Nursing management
Dysrhythmias:
Bradycardia
Progression of AV-block
Electrolyte imbalances:
Hypokalemia may lead to digitalis toxicity
**both fight for same receptors
Digoxin toxicity s/s
Early toxic s/s
Green/yellow vision
Double vision
Seeing spots or halos
HA
Dizziness
Fatigue
Weakness
Digitalis toxicity
Tx
Hold next dose and wait until 1/2 life is reached then:
d/c or lower does
Atropine is first line DOC to tx symptomatic bradycardia
Antidote DIGIBIND is utilized
*hypokalemia is reversed first, dysrhythmias, reversal of +inotropic may take several hours

Milrinone
