Principles

Stage 1
Teaching treatment components
overview, introduce severity rating scale, learn to control fluency with the training model (technique of 7-8 in spontaneous speech)
3-5x 45-60 minute individual sessions
Stage 1 progression criteria
self assign scores using SR scale that are similar to clinicians
consistently use fluency technique similar to training model with SR 0 and technique rating 7-8
feel in control
recognise fluency technique scores
Subjective units of distress scale SUDS
useful to monitor anxiety
record anxiety level along with SR and fluency technique scores
Stage 2
Establish stutter-free speech
- consolidate imitation of training model fluency technique
- work with clinical to develop an individualised, natural-sounding technique that is acceptable for stuttering control
- Practice self-evaluations
- develop problem-solving
- encouraged to use technique in everyday speaking
Stage 2 progression criteria
- individualised technique to control stuttering has been developed
- stage 3 commences when client can speak with the clinician for the entire session at SR 0-1 with fluency technique score acceptable to the client
Stage 3
Generalisation
devise hierarchy of speech situations to assist transfer
plan treatment changes for following week based on speech measures
modify measurement procedures if required
establish a routine, motivate and aid generalisation
Stage 3 progression criteria
- stuttering and technique goals are met for 3 consecutive weekly consultations
- goals for SR at 0-1 in most everyday speaking situations
- no situation avoidance and acceptable fluency technique score according to client
Stage 4
Maintenance of treatment gains
- maintain stutter-free speech through consultation
acceptable SR scores and technique scores in everyday speaking situations
- confirmed scores with audio recordings
- demonstrate how variations In SR have been dealt with appropriately
- Booster programs
- self help groups
Stage 4 progression criteria
Discharge will be negotiated once demonstrating:
- skills for monitoring speech and controlling stuttering
- ability to address fluctuations in stuttering severity
- achievement of personalised goals for stuttering treatment
Weaknesses
no programmed instruction which may be required for more severe cases
high motivation required
doesn’t deal with self esteem issues
requires training for clinician
