Features of autistic spectrum disorder?
Deficits in social interaction, communication and behaviour. Typically evident before age 2-3 years.
Social interaction - lack of eye contact, delay in smiling, avoids physical contact, unable to read non-verbal cues, difficulty establishing friendships.
Communication - delay/absence or regression in language development, difficulty with imaginative behaviour, repetitive use of words.
Behaviour - general interest in numbers/patterns. Self stimulating movements, niche interests, extremely resitrcted food preferences
Diagnosis and management of ASD?
Ix - Paediatric psychiatrist. Can be done before age 3.
Rx - MDT with child psychology, speech and language specialists, dietitian.
Features of ADHD?
Very short attention spam,
Quickly moving from one activity to another,
Quickly losing interest in tasks,
Constantly moving,
Impulsive behavior,
Disruptive or rule breaking.
Management of ADHD?
Conservative - healthy diet and exercise.
Medication - Methylphenidate, dexamfetamine, atomoxetine.
Possible blood results in restrictive eating disorders?
Anaemia,
Leucopenia,
Thrombocytopenia,
Hypokalaemia
Features of refeeding syndrome?
Hypomagnesaemia, hypokalaemia, hypophosphataemia, fluid overload.
Risk of arrhthmia and heart failure.
Features of bulimia nervosa?
Erosion of teeth,
Swollen salivary glands,
Mouth ulcers,
GORD,
Calluses on knuckles (russell’s sign)
Features of anorexia nervosa?
Weight loss,
Amenorrhoea,
Lanugo hair,
Hypotension,
Hypothermia,
Mood changes.
High risk of cardiac complications - arrhythmias, cardiac atrophy, sudden cardiac death.
What is tourette’s syndrome?
Development of tics that are persistent for over a year. Often presents around age 5.
Examples of simple and complex tics?
Simple - clearing throat, blinking, head jerking, sniffing, grunting.
Complex - Physical movements, copropraxia (obscene gestures), coprolalia (saying obscene words), Echolalia (repeating other peoples words).
Management of tourettes?
Mild - reassurance.
Severe - habit reversal training, exposure with response prevention and meds if very severe - Clonidine/guanfacine (alpha 2 adrenergic agonists) or antipsychotic meds
