Step 1: Fasting Lab
Not required to measure total cholesterol or HDL
Fasting Required for measurement of triglycerides and a
calculated LDL
9 to 12 hour fast

cLDL=TC-HDL-(Trig/5)
only works if TRIG <400mg/dL
TC 266 mg/dL, Trig 177 mg/dL, HDL 35mg/dL=196mg/dL
TC 178 mg/dL, Trig 100 mg/dL, HDL 46mg/dL=112mg/dL
TC 180mg/dL, Trig 500mg/dL, HDL 80mg/dL=0 (the triglycerides are too high its over 400)
When pt is a new and not on a statin and your trying to figure out whether they need a statin because ultimately statins are the best you want a fasting lab…the only lab for cholesterol that is influenced by food are the triglycerides…we don’t measure LDL we calculate it so if you look at labs it little c LDL and its calculated LDL
Total cholesterol and HDL if your just measuring those you don’t need a fasting lab you can come in and do it anytime but if your looking for triglycerides and if you need to calculate the LDL then you need the fasting labs pt can drink water and take their meds but they can’t eat for 9 to 12 hours and its calculated from the Friedwald there’s a newer calculation so this tends to get inaccurate as the triglycerides get high and it gets inaccurate as LDL gets really low so pts on PSK9s
When you have triglycerides that are over 400 you need to measure the LDL directly…that would be an additional lab and they don’t have to fast for that
Only cholesterol test that has to be fasting is the triglycerides but because we use that in this calculation we need it for the calculation but direct LDL,total cholesterol, and HDL is not influenced by food
