Blocks calcium-channels to reduce force of contraction, conductivity and vascular tone.
Dihydropyridine CCBs - MoA
Vasodilation = relaxation of the peripheral blood vessels.

Dihydropyridine CCBs - side effects
- Headache (common)
- Ankle swelling
- Flushing
- Palpitation
Non-dihydropyridine CCBs - MoA
Exert effects on cardiac muscle.
Non-dihydropyridine CCBs - examples
- Verapamil
- Diltiazem
Verapamil
Causes constipation
Only CCB licensed for arrhythmias
Less commonly causes:
- Bradycardia
- Heart block
- Cardiac failure
Diltiazem
> 60mg = maintain on same brand
Interactions
Grapefruit juice (enzyme inhibitor) - increased CCB concentrations.
Beta blockers + non-dihydropyridine CCBs = heart failure, bradycardia + asystole
Non-dihydropyridine CCBs - caution
Impaired left ventricular function
Avoid in unstable angina + severe aortic stenosis
