Pathology of slow narrowing
A
Atherosclerosis of coronaries- Rigid, calcified. Narrowed due to fibrotic plaques & atheromas. Ca++ salt & Cholesterol deposits present.

Pathology of acute thrombosis
A
MC in arteries already narrowed by atherosclerosis
Sudden occlusion=MI

Location of occlusion
A
- 50% LAD–Anterior wall LV
- 30-40% RCA–RV & Posterior wall LV
- 10-20% L circumflex–Lateral/Free wall LV

Acute MI causes
A
Sudden occlusion by:
- Thrombosis (80-90% of transmural MI)
- Ulceration of embolized plaque
- Prolonged vasospasm

Cause of sudden death after MI
A
25% sudden death
- Most=arrhythmias (VFib)
- Complete heart block
- CHF later on

Complications if survive
A
75% survive
1. CHF
2. Cardiogenic shock–decreased perfusion–multiorgan failure
Most dangerous=Cerebral ischemia

Types of MI
A
- Transmural=all 3 layers
Involves free wall LV &/or septum
3 complications - Subendocardial/Intramural=concentric around subendocardial layer
1/2 of the tissue layer is still viable

Complications of transmural MI
A
- LV aneurysm
- LV rupture
- Mural thrombus–CVA
