What is Parkland formula for burns?
it is 4cc(weight)(total body surface area)
Give first half in first 8 hours, the second half in next 16 hours.
What is added in IV for burn victims?
Ascorbic Acid. This decreases the overall burn injury.
Also vit C is antioxidant - and it is presumed that a significant amount of ORGAN damage is caused by cytokine release of free rad’s
What is the best measure of resuscitation of burn victim?
Urine Output is what is measured.
peds = 1cc/kg/hour
adult = .5cc/kg/hour
Raw equation of mortality for burn victims
TBSA + Age correlates with mortality.
What is a first degree burn?
sunburn, erythema, swelling,
bsaically anything that turns you red.
Second degree burn? What to do to them?
blisters. gets into dermis.
You will want to pop any clear blisters.
Leave hemorrhagic blisters alone.
Third degree burn?
Painless becuase burns nerves. penetrates into subcutaneous. Needs grafting.
Fourth Degree?
Penetrates deep muscle, bone, organs.
Severe and life threatening.
Slivadene? What is a common complication that comes with it?
Silvadene.
has silver ions in it.
It can cause neutropenia (low WBC). once it’s use is discontinued - WBC’s go back fine.
Mafenide Acetate? Complications?
carbonic anhydrase inhibitor i.e. metabolic acidosis
Silver nitrate? Complications?
leaches Na+, K+, Ca2+, Cl–
So hypo- of all of these.
Why is lots of fluid given with electrical injury?
Because electrical injury will burn muscle/release stuff that kidney needs to clear.
So need more fluids to clear it.
At what temperature should you use to reheat frozen lumbs/frostbite?
You want to use warm at 104 degrees Fahrenheit.
What kind of injury is caused by acid? How to treat it?
Coagulation necrosis. Just use water.
How to treat HF (hydroflouric acid) burn?
Treat with CaGluconate and Mg2+
Can be subcu, topical, and intraarterial,
treat until pain goes away.
