A

Significant burn injury in Pts w/ pre-existing medical disorders
that could complicate management, prolong recovery, or affect
mortality, such as:

  • Diabetes
  • Cardiac disease
  • Pulmonary disorders
  • Pregnancy
  • Cirrhosis
  • Morbid obesity
  • Immunosuppression
  • Bleeding disorders

Initial Actions/Considerations

A
  • General Impression
  • LOC
  • ABC’s (O2, Vent)/Consider Airway Management if suspected
  • Remove Clothes, Jewelry, or other constrictive Items
  • Keep Warm
  • HX & Vitals
  • Determined Burn TBSA or Circumferential Burns
  • Consider Abuse (Elderly/Ped)
  • No IM Pain Meds thru Burnt skin
  • If Electrical in Nature, ensure scene safety 1st & anticipate arrhythmias (3 lead)
  • Lighting = Reverse Triage

  • 2 Large Bore IV/IO Access & LR (if >10%)
  • Leave Blisters intact & cover w/ a Burn Sheet/Dry Sterile Dressing/Clean Sheet
  • Pain management (Ketamine)
  • Cardiac Monitor & End Tidal
  • Calcium & Albuterol (Compartment Syndrome)
  • Provide Direct Etele & Rapid Transport
