- specify info needed
- specify type of interviewing method (mail/personal/telephone)
- determine content of questions
- design the questions to overcome respondent’s inability + unwillingness
- decide question structure (structured vs unstructured)
- determine the question wording (6 Ws)
- arrange the questions in a proper order
- identify form + layout
- reproduce the questionnaire
- eliminate bugs by pre-testing
