-concurrent strong P-Gp/CYP3A4 inducer/inhibitor or persistent need for apt
-patient choice

Answer 2

When the following present:
- luminal cancer (gi, urothelial, intracranial)
-gi tract impairment (n&v, gi toxic sact, sb resection, high output stomach or gastrectomy)
-crcl <30
-concurrent strong P-Gp/CYP3A4 inducer/inhibitor or persistent need for apt
