What are common pathogens involved in bronchiectasis?
H.influenzae
Strep
Staph
Pseudomonas
Congenital causes?
CF
PCD/Kartagener’s
Young’s syndrome
Hypogammaglobulinemia
Acquired causes?
Idiopathic
Post-infectious e.g. measles, pertussis, TB, HIV
Bronchial obstruction e.g. tumour, foreign body
ABPA
RA, UC
Tests
Sputum culture PEFR Bloods- anemia of chronic disease, if aspergillus then RAST, precipitins, eosinophilia, if RA then anti-CCP, RF, ANA Imaging (CXR, HRCT) Spirometry: obstructive disease
Findings on CXR and HRCT?
CXR: tramlines, ring shadows (bunch of grapes)
HRCT: signet ring sign, pools of mucus in saccular dilatations
Symptoms?
Cough Purulent sputum Dyspnoea Haemoptysis Weight loss/cachexia
Treatment is split up into conservative, medical and surgical
Conservative: MDT, physio
Medical: Abx, bronchodilators, treating underlying cause, vaccinations
Surgical: may be indicated in severe localised disease or obstruction
Complications
Cachexia Pulmonary HTN & cor pulmonale Massive haemoptysis Type 2 respiratory failure Amyloidosis
Blood tests important for CF?
FBC U&E LFT Clotting Annual glucose tolerance test Vitamins ADEK
