HOw should you discuss a code status in a terminally ill patient?
- clarify patient prognosis
- -goals (Die at home w/ family, in hospital) - code procedure
- -risks & benefits
- includes CPR, defib, meds, intubation - recommendation

physician order for life sustaining treatment
if you are going to die w/i 5 years
fill this out & give all specifics so that physicians know what to do.
