criterion A for brief psychotic disorder
presence of at least ONE of the following symptoms:
delusions
hallucinations
disorganized speech
grossly disorganized or catatonic behaviour
at least ONE of these must be one of the first three (I.e cant just have grossly disorganized/catatonic behaviour)
criterion B brief psychotic disorder
duration of an episode of disturbance is at least ONE DAY but less than ONE MONTH with eventual RETURN TO premorbid level of functioning
criterion C brief psychotic disorder
disturbance not better explained by major depressive or bipolar disorder with psychotic features or another psychotic disorder such as schizophrenia or catatonia and is not attributable to the physiologic effects of a substance or another medical condition

- with marked stressor(s) (brief reactive psychosis)
- without marked stressor(s)
- with post partum onset
- with catatonia

use this if symptoms occur in response to events that, singly or together, would be markedly stressful to almost anyone in similar circumstances in the individuals culture

what is the essential feature of brief psychotic disorder
disturbance that involves SUDDEN ONSET of at least one of the positive symptoms of psychosis
sudden onset = change from nonpsychotic state to clearly psychotic state WITHIN TWO WEEKS
usually WITHOUT prodrome
prevalence of brief psychotic disorder
may account for 9% of cases of first onset psychosis
is there a gender difference in prevalence of brief psychotic disorder
yes–> twofold more common in females than males
what is the average age of onset of brief psychotic disorder
mid 30s
can occur across lifespan and may appear in adolescence or early adulthood
what might predispose someone to brief psychotic disorder
pre-existing personality disorders and traits i.e schizotypal or borderline PDs or traits in the psychoticism domain i.e perceptual dysregulation and the negative affectivity domain i.e suspiciousness may predispose to brief psychotic disorder
what is the prognosis for brief psychotic disorder
high rates of relapse
BUT still outcome is excellent for most in terms of social functioning and symptomatology
what is the first line treatment for brief psychotic disorder
antipsychotics–> particularly second generation
how long should you treat brief psychotic disorder with antipsychotics
even though symptoms by definition resolve within 1 month, recommend treating 1-3 months after symptom resolution
which second gen antipsychotic may be more preferable in lactating mothers

what are positive prognostic factors for brief psychotic disorder
absence of genetically related individuals with schizophrenia or brief psychotic disorder
sudden symptom onset
presence of stressful triggers
short duration of symptoms
