What is a breech birth?
Presentation of the bottom first into the pelvic inlet
What are the main two factors that contribute to a breech presentation?
Endogenous - inability for fotal movement
Exogenous - insufficient uterine space
What are the 3 types of breech?
Based on how the legs are lying
Frank breech - legs are up to abdomen
Complete breech - baby appears as it’s sitting with it’s legs crossed
Footling - where one or both feet are born before the pelvis
What are the complications of breech presentations?
- Cord Prolapse
- ## Head can compress the cord when at the pelvis
What in relation to the arms and head will not allow the birth to happen? How can this be managed?
When the arms are at the side to the head,
Lovset manoeuvre
What is involved in the Lovset Manoeuvre?
- Rotating the fotal body
- This causes the arm to tail downwards infront of the face
What is the consequence of a rapid birth and why?
A rapid birth can cause compression and decompression of the head cause brain damage. This is abnormal as there is gradual molding in a cephalic birth. however in breech there isn’t that gradual process.
What is used to manage a birth?
Mariceau-Smellie-Veit Manoeuvre which uses elective application of forceps.
What is the management of breech presentation?
C-section
External Cephalic Version
What is the managment for breech presentation of both twins?
C-section
Breech Extraction
What is breech extraction
procedure that involves the obstetrician grabbing the second twin’s feet and pulling him/her into the birth canal. This will help with delivering the second twin vaginally
When is an external cephalic version done?
36-40 weeks
