What things are apart of the breast health history?
breast self exam practices, last menstration, freq. of clinical breast exams, recent of past mammo in women older than 40, personal or fam hx of bc, hx of BRCA1 & BRCA2 testing, past or current lunps, pain/discomfort, and discharge (not breastfeeding)
During the inspection of the breast, what are you looking for?
asymmetry, edema, nipple retraction, redness, color changes in breast or nipples
During palpation of breast, what do you palpate for?
lumps, changes in tissue consistency, nipple discharge, tenderness, or restricted mobility in breasts and axillary region
What are some common or concerning subjective or objective findings?
hx of breast mass, breast pain/discomfort, nipple discharge in the absence of recent birth/breastfeeding, nipple retraction, edema not related to breastfeeding, enlarged lymph nodes bilaterally
What are the different positions that a pt. could be in?
sitting with arms at rest, arms over head, arms pressed into hips, leaning forwards AND lying with arms over head
When you inspect the nipple, what should you look for?
Inspect for nipple retraction or inversion, redness, and discharge from nipple
What is inversion?
absence of nipple erection reflex
What kind of nipple discharge can you have?
serous (clear fluid), green, yellow, or bloody
What are the findings of Erythem and edema of breast?
redness, change in breast contour in lateral aspect of breast, prominence of pores, and loss of distinct borders or areola and nipple
What kind of patterns can you use to palpate the breast?
vertical or concentric circle pattern
When you palpate the breast, what are you looking for?
changes in consistency, tenderness, nodules, size differences, shape differences, decreased mobility with patient position change, and changes in lymph nodes in axillary region
What do you evaluate in a breast nodule?
location, size, shape, consitency, borders, tenderness, and mobility/fixed
What is fibroadenoma?
single grouped, round disclike, soft or firm, well defined borders, mobile, nontender, no redness, no edema, no retractions, and located in any quadrant of breast
What are cysts?
single or multiple grouped or throughout breast tissue, round, soft, firm, “rubbery”, well defined borders, mobile, usually tender, no retractions, no edema, no redness, and any quadrant
What is breast carcinoma?
more common 50+, usually solitary, firm or hard, confluent with surrounding tissue but may have well defined borders, nontender, nipple retraction, blood discharge, edema, dimpling, and enlarged lymph nodes.
