Stage 1 or 2 HTN
S1 - CBPM over 140/90 and ABPM/HBPM over 135/85
S2 - CBPM over 160/100 and ABPM/HBPM over 150/95
Indications to prescribe
All S2 or 3 HTN
People with S1 who have one or more of following:
- Target organ damage
- Established CVD
- Renal disease
- Diabetes
- 10 year CV risk equivalent to 20% or ge
How is CV risk estimated?
ASSIGN
Based on age, BP, weight/height, gender, smoking, cholesterol, ethnicity, social class, family hx, diabetes/RA/renal function
How is MAP measured?
TPR x CO
ACE/ARB CI’s
Angioneurmtic oedema
Renal artery stenosis
Pregnancy
CCBs
Block entry of calcium through slow channels in cardiac and smooth muscle
Reduce CO
Dipyrimidine CI’s
Uncontrolled HF
Cardiogenic shock
Significant AS
Unstable angina
How do thiazides and aldosterone ants work?
Thiazides - inhibits Na/Cl channels in DCT
Spironolactone - Na/Cl inhibits channels in CT
