most common benign bone tumor, cartilaginous origin, appears as subungual exostosis

epiphyseal lytic lesion, younger patients
Enchondroma
cartilaginous, central/medullary, thin rim of sclerosis, mostly phalanges, expansive lesion with fleck-like calcifications/”stippling”

OSTEOID OSTEOMA
– X-ray nidus «_space;1.5 cm
* Sclerotic margin
* Lucency in the middle
OSTEOBLASTOMA
Arise from osteoid
- Occurs in axial skeleton
- Nocturnal pain NOT relived with Aspirin
- HALLMARK – X-ray nidus»_space; 1.5 cm
Fibrous dysplasia
ground glass appearance, cortical expansion of long bones
Non-ossifying fibroma
rgresses spontaneously
asymptomatic, lobulated coarse trabeculation, Jaffe-Campanucci syndrome(cafe au laiot spots, cardio)
Unicameral bone cyst
“fallen fragment” sign, high recurrence after surgery
Giant cell tumor
cortical expansion, “soap bubble” appearance, high correlation with Paget’s disease
Aneurysmal bone cyst
vascular malformation, metaphyseal, soap bubble pattern, benign but appears locally aggressive, rapid onset of pain/swelling
Pigmented villonodular synovitis
inflamed synovium, mono-articular (knee)
hyperparathyroidism
Brown tumor
Fibrous cortical defect
metaphyseal, inner cortical convex “bubbling”
Chondromyxoid fibroma
dramatic expansion with/without cortical resorption, eccentric oval geographic lesion
Enostosis
, no periosteal reaction/destruction/cortical expansion, “brush border”, bone island/bone within bone
Multiple myeloma
most common malignant bone tumor in body, lytic “punched out” lesion, Bence-Jones protein, axial skeleton
Osteosarcoma
“moth eaten, permeative”, soft tissue invasion, “sunburst” or “hair on end” destructive pattern, bone pain, mets to lungs
Ewing’s sarcoma
from bone marrow, diaphyseal, children, “onion skin” periosteal reaction, femur and spine, pathologic fractures. Diaphyseal.
Chondrosarcoma
lytic lesion with poorly defined margins, most common malignant tumor of foot in adults, endosteal scalloping, pure hyaline cartilahe, fluffy or ring and arc. dull pain and slow growing
Fibrosarcoma
“permeative or moth-eaten”, not as fast in its destruction as osteosarcoma. NO periosteal reaction, amputation.
