Bone physiology Flashcards
1
Q
Paget’s disease: what
A
- Disorder of bone remodelling
- Uncontrolled turnover leading to bone remodelling, enlargement, deformity, weakness
- Common in Europe and with older age
2
Q
Paget’s disease: symptoms
A
- Asymptomatic 70%
- Bone pain and deformity - pelvis, lower limb bones, lumbar spine, skull
- Pathological fractures
- OA
- Deafness/nerve compression
- Cardiac failure
3
Q
Paget’s disease: biochemistry
A
- High ALP
- Normal Ca+ and PO4
4
Q
Paget’s: management
A
- Analgesia
- Alendronate
5
Q
Osteomalacia: what (bone and mineral content)
A
- Normal amount of bone BUT
- Low mineral content
… Called Rickets if happens during bone growth (younger years)
6
Q
What are the metabolic bone diseases?
A
OP
Osteomalacia/Rickets
Paget’s
7
Q
Osteoporosis: what (bone and mineral content)
A
- Bone loss BUT
- Normal amount of minerals
8
Q
Osteomalacia: symptoms
A
- Bone pain
- Fractures
- Symptoms of low PO4 or Vit D
9
Q
Osteomalacia: causes
A
- Low Vit D (malabsorbed, poor intake, no sunlight)
- Renal failure or liver disease (affect Vit D levels)
- Drugs (affect Vit D levels)
10
Q
Osteomalacia: biochemistry
A
- Low Ca2+, PO4, Vit D
- High ALP and PTH
11
Q
Osteomalacia: management
A
- Vitamin D (different variations depending on the cause)
12
Q
Osteoporosis: what
A
- Reduced bone mass
- 1o (age) or 2o (drugs, etc)
- Affects trabecular bone: vertebral crush fractures
… and cortical bone: NOF
13
Q
Osteoporosis: risk factors
A
- Parental history
- Alcohol, smoking
- RA
- Low BMI
- Early menopause
- Steroids
- Renal or liver failure
14

Osteoporosis: investigations
A
Bone densitometry, FRAX
Ca, PO4, ALP normal
15
Q
Osteoporosis: management
A
Bisphosphonates
Calcium and Vit D if deficient
Denosumab (monoclonal Ab)
