What are indications of plain film abdomen?
Evaluate bowel gas pattern: obstruction vs. ileus
Evaluate for calcifications: kidney, ureter, GB, appy
Evaluate for free air or pneumatosis
what is dilated more in ileus? sb or lb?
both are dilated equally although large is bigger
what are the common calcifications seen on abdominal XR?
renal stones
gallstones
appendicolith
what do we get porcelain gallbladder from?
chronic cholecystitus and it needs to be removed
What is Rigler’s sign and what does it suggest?
air on both sides of bowel–means abdominal free air
What is the football sign?
falciform ligament outlined by air- means free air
what happens to liver with pneumoperitoneum?
becomes more lucent and can become outlines—confirm on decubitus
What is pneumatosis?
Gas located in subserosa +/- submucosa with normal mucosa and muscularis
What are the common causes of pneumatosis?
ischemia- NEC, mesenteric vascular disease
iatrogenic trauma
Infection, inflammation/IBD
In abdominal trauma, is plain film often used? what do we use?
no- rarely
FAST scan (for fluid), CT (contusion, laceration, rupture, fractures)
What 4 areas do we assess with FAST scan?
perihepatic
pelvis
perisplenic
pericardium
characteristics of appendicitis imaging include?
> 10mm
fat stranding
appendicolith
on US, what is characterisitc of a stone?
echogenicity with a shado
a stone in a hollow organ with brightness surrounding it, is what?
acute cholecystitis
Huge gray donut around a white hole might be?
