Blood Transfusions and Blood products Flashcards
RhoGAM
Given intramuscularly at 28 weeks gestation and wihtin 72hr of birth, prevent mother from developing Rh+ antibodies that could affect future pregnancies
Autologous Transfusion
Collection of client’s own blood
Up to 5 weeks before surgery
What gauge catheter for blood products?
18-22 unless rapid (16-18)
What is required in administration of blood products (within the supplies?)
Y-type tubing
Micron Filter
When should administration be complete?
within 4hr of starting
Signs of transfusion reactions?
Fever w or w/o chills
tachycardia, dyspnea
Hypotension
Hives/rash/pruritis
Pain
Types of Transfusion Reactions
Acute hemolytic (5-30min)
Febrile Non-hemolytic (30min)
Allergic (anytime)
Anaphylactic (5-15 min)
What is TPN?
Total parenteral nutrition
Nutrients directly to bloodstream
Prepped by pharmacy
TPN = High risk for…
Infection, high glucose increases risk
What type of fluids are TPN?
Hypertonic
10-15% dextrose in water w additives
How often should you check bg with TPN?
q6h
Complications of TPN
Pneumothorax
Catheter occlusion
infection
Hyper/hypoglycemia
When are electronic infusion devices used?
Low hourly volumes (<20ml/hr)
High hourly volumes
Pts at risk of fluid overload
Examples of blood products derived from plasma
Albumin, intravenous immune globulin, rh immune globulin etc
What to do if suspecting a transfusion reaction
Immediately stopping the transfusion
removing tubing
replace line with normal saline
what is hemolysis
Breakdown of RBC’s
Blood must be started within..
30 min of accessing it from lab
First step before administering blood products
Check baseline vitals
acute intravascular hemolytic reaction
ABO-incompatible blood products
Antigens react
Chills, low bp, tachycardia, back pain
Febrile nonhemolytic reaction
Antibodies against donor WBC
Most common
Sudden chills, fever, headache, flushing
Mild Allergic
Antibodies against donor plasma proteins
flushing, itching, hives
Anaphylactic reaction
Antibodies to donor plasma
Anxiety, wheezing, cyanosis, hypotension
