Haemolytic disease of the new born (HDN)

How does it occur?

Which class of abs can cross placenta?

A

A person may from RBC abs through blood transfusion, or if fetal cells enter maternal circ during preg/delivery

Maternal ab high > destroy fetal RBCs > fetal anaemia + jaundice

only IgG can cross placenta

Ab most often responsible is anti-D > so give RhD -ve blood to RhD negative woman of child bearing age

Other abs - anti-c/K, IgG ABO

When should you give a mother Anti-D Ig? (3)

A

High risk of feto-maternal haemorrhage

Routine antenal prophylaxis

during pregnancy if sensitizing event occur e.g. misccarage, amniocentesis

at delivery if baby is RhD +ve
