What is the Blood Injection Injury Invetory (BIIPI)?
An 18-item self-report measure used to assess the severity of fear related to blood, injections and injuries.
How is the BIIPI set up?
- The patient is presented with a situation.
- They have to evaluate the different reactions they may experience in that situation (cognitive, physiological, behavioural reactions) on a 0-3 rating scale.

How is the BIIPI scored?
- Items/situations rated on a 0-3 rating scale.
- 0=Never
- 1=Sometimes
- 2=Almost always
- 3=Always

Can you give an example of an item on the BIPI?
- Situation: When I see blood on my arm or finger after pricking myself with a needle.
- Examples response: ‘I think I might faint’ 0-3 rating scale.
Name 2 strengths and 2 weaknesses of the BIPI
Strengths= Holistic and comprehensive. Clinical utility.
Weakness= Self-report bias. Limited understanding of causes.
Explain 1 strength of the BIPI relating to holism?
- BIPI= Comprehensive as it measures fear + evalutes cognitive, physiological,behavioural responses.
- Holistic approach= Useful for understanding people who may experience a 2 phase response (anxiety-fainting)
two-phase response—an initial spike in anxiety followed by a sudden drop in blood pressure leading to fainting.
Explain another strength of the BIPI- relating to clinical utility.
- Identification- Helps identify who need treatment due to symptom severity
- Tracking- Can track improvements over time (particularly in exposure therapy/ CBT)
- Treatment planning- Can aid in treatment planning by highlighting specific triggers (some may fear needles but not blood, while others fear both).
Explain one weakness of the BIPI relating to self-report bias.
- Relies on self-awareness, honesty and mood of day they completed the questionnaire.
- Some may overestimate their fear (attention) or underestimate (shame).
- Reducing the accuracy- lowering validity.
Explain 1 weakness of the BIPI relating to limited understanding/reductionism
- Provides limited understanding of underlying causes or individual differences. (trauma-witnessing severe injury, family history of BII- genetic predisposition)
What is the aim of the BIPI?
- DIAGNOSE individuals who experience EXTREME physiological reactions when exposed to such stimuli
