Bleeding Disorders Flashcards
Name some acquired bleeding disorders
Reduced platelets - immune thrombo cytopenia and leukaemia
Liver disease
Kidney disease
Anticoagulant or antiplatelet medication
Name some inherited bleeding disorders
Von willebrands disease
Heamophillia A
What is the function of tranexamic acid?
Tranexamic acid is a medication prescribed in several bleeding disorders due to its ability to reduce clot breakdown
What is immune thrombocytopenia?
Immune mediated destruction of platelets due to autoantibody production leading to reduced platelet number, causing abnormal primary haemostasis
Why is bleeding time affected in patients with liver disease?
Because the liver is responsible for the production of clotting factors so in liver disease this function will be lost or impaired. They will also have reduced platelet count. This means the coagulation pathway and primary haemostasis are affected.
Why are aspirin and clopidogrel relevant to bleeding disorders?
They are both antiplatelet medications, primarily prescribed to prevent arterial thrombosis. They inhibit platelet activation so bleeding time will be longer. Can overcome the effect by stopping the medication for a week
WARFARIN GO!!!
Oral anticoagulant used in the long term prevention of venous and arterial thrombosis.
Inhibits the vitamin K dependent clotting factors (2,7,9,10)
Dose adjusted so INR between 2 and 3
Blood test every few weeks to adjust dose accordingly as the required dose can be affected by other drugs and dietary changes
Heparin
Is a short term anticoagulant used in urgent treatment or during pregnancy
Mainly used in hospital
Inhibits coagulation factors
What are the drug interactions with warfarin?
Metronidazole (daktarin oral gel) - will increase the anticoagulant effect of warfarin
Amoxicillin and erythromycin
NSAIDS - double blood thinning
What is haemophilia?
Genetic bleeding disorder, there are several types.
A ➡️ factor 8 deficient
B ➡️ factor 9 deficient
C ➡️ factor 11 deficient
