what is Bishop score
The Bishop score is a scoring system used to determine whether to induce labour.
indications for induction of labour
prolonged pregnancy
PPROM
obstetric cholestasis
diabetes > 38w
Interpretation
a score of < 5 indicates that labour is unlikely to start without induction
a score of ≥ 8 indicates that the cervix is ripe, or ‘favourable’ - there is a high chance of spontaneous labour,
components of bishop score
Call PEDS For Parturition
Cervical - position, effacement, dilatation, softness, fetal station
position - posterior , intermediate, anterior
effacement - 0-30,40-50,60-70, 80
dilatation - <1,1-2. 3-4, >5
softness- form, intermediate, soft
fetal station -3,-2,-1 to 0, +1,+2
induction of labour based on bishop score
<6 : vaginal prostaglandins/ oral misoprostol
>6 : amniotomy + IV oxytoxin
complications of Induction
uterine hyperstimulation
prolonged and infrequent uterine uterine contractions
interruption of blood flow which can cause uterine rupture, fetal hypoxia
management of uterine hyperstimulation
remove prostaglandins, consider tocolysis
