Mania
What it is?
Criteria for DSM-5
Extreme euphoria ( extremely happy, elated, increased energy)
- Inflated self esteem or grandiosity ( grandiose delusion, persecutory delusion- someone is out to get them)
- Decreased need for sleep
- Pressured Sleep ( person keeps talking and talking )
- FLIGHT OF IDEAS= racing thoughts ( when they jump form one topic to the next )
- Increasing goal-directed activity ( work, school, heypersexuality )
- Excessive involvement in unsafe activities ( sex, drugs, driving fast )
Bipolar 1 Disorder
** Has EXTREME MANIA + SEVERE DEPRESSION **
- full syndrome of manic symptoms
- psychosis is possible
- HIGH RISK FOR SUICIDE
Bipolar 2 Disorder
** Has SEVERE DEPRESSION + HYPOMANIA **
- Has never met criteria for full manic episode
Cyclothymic Disorder
** Has HYPOMANIA + MILD/ MODERATE DEPRESSION **
Mania risk factors
excess of norepinephrine and dopamine in the brain
strong hereditary implications
Lithium expected side effects + therapeutic range
Expected Side effects:
-drowsiness, HA, dry mouth, polydipsia, fine hand tremors, polyuria, weight gain
Therapeutic range: 0.6-1.2
- monitor levels 1-2x a week till stable then monthly
Lithium toxicity levels
Early sx at 1.5-2= coarse hand tremors
2-2.5= Patient must be hospitalized
>2.5= death, pt will likely need hemodialysis
