what is bipolar classified by?
presence of manic or hypomanic episodes
euthymic:
when a bipolar individual is in between episode but not experiencing symptoms of mania or depression
depressive episode in BP compared to MDD:
consist of more mood lability, more psychotic features, more psychomotor retardation, more substance abuse
bipolar depressive episodes tend to be more ___ than MDD episodes:
severe
building blocks of bipolar:
manic episodes, hypomanic episodes, depressive episodes
diagnostic criteria for manic and hypomanic episodes:
during period of mood disturbance and increased energy or activity, 3 or more of following symptoms and represent a noticeable change from usual behavior
symptoms of manic episodes:
- inflated self esteem or grandiosity
- decreased need for sleep
- more talkative than usual
- racing thoughts
- distractibility
- increase in goal-directed activity
- excessive involvement in activities that have high potential for painful consequences
Differences between manic and hypomanic episodes:
duration of symptoms and impairment in one’s life
duration of symptoms in manic episodes:
lasting at least 1 week and present most of the day, nearly everyday
duration of symptoms in hypomanic episodes
lasting at least 4 consecutive days and present most of the day, nearly everyday
Impairment in one’s life during manic episode:
severe enough to cause marked impairment in social or occupational functioning or to need hospitalization to present harm to self or others, or psychotic features
Impairment in one’s life during hypomanic episode:
not severe enough to cause social and occupational functioning, to to necessitate hospitalization or harm or psychotic features. disturbance in mood is observable by others and maybe not so much by individual
bipolar 1 diagnosis:
person has manic episode and can also have hypomania, can also ave depression but does not need it. manic episode is mainly was classifies bipolar 1
bipolar 2 diagnosis:
must have hypomania and depressive episode
Cyclothymic diagnostic criteria:
for at least 2 years there have even hypomanic symptoms that do not meet criteria for hypomanic episode and periods of depressive symptoms that do not meet criteria for major depressive disorder. during 2 years, periods have been present for at least half the time and individual has not been without symptoms for more than 2 months.
diagnostic criteria for other specified bipolar disorder:
if you have full-threshold hypomanic episodes but no full threshold depressive episodes
mixed features specifier in mania:
depressed mood, loss of interest, depressive-like
rapid cycling:
presence of at last four mood episodes in the previous year that meet criteria. episodes must be followed by either a period of full remission of switch to an episode of the opposite polarity
in full remission:
2 months symptom free
in partial remission:
symptoms of previous episode is present but full criteria not met, or period lasting less than 2 months without any significant symptoms
ultra rapid cycling:
switches between states in the magnitude of days-weeks
ultra-ultra rapid cycling (ultradian)
switches between states in the magnitude of hours-days
diagnostic naming of bipolar:
name of bipolar disorder, name of current or most recent episode, severity/remission specifiers where applicable
