When will the physician write an order to refer the patient to BCCA/FVCC?
When they are newly diagnosed with cancer or require a follow-up after initial consult/appointment.
Where does the NUC transcribe the BCCA order?
On the Kardex under the “treatment” section
What information does the NUC complete on the BCCA ref form?
Addressograph/label and fax stamp. The Physician or RN will complete.
What is needed when referring a patient to the BCCA?
- All pathology/cytology reports
- Operative reports
- All x-ray and scan reports pertaining to the patient’s diagnosis of cancer
- All consultation reports
- Any specialized tests
How does the NUC arrange transport?
Arrangements need to be made with BC Ambulance, Transport Service or a family member.
What does the NUC check for upon the patients return? Where do they put it?
BCCA appointment card.
Transcribe the next appointment on the Kardex and book future appointments prn, then attach the card to the inside of patients chart.
What information is required when booking patient transport?
- name of patient
- DOB
- PHN
- gender
- pick up facility and ward and telephone #
- drop off facility and ward and telephone #
- date and time of appointment/return time
- reason for transfer
- what procedure is being done
- Any monitoring required
- contact precautions
- weight of patient
- special equipment
- escort information
- if going to a clinic/doctors office will the stretcher fit in the waiting room
