A

Addressograph/label and fax stamp. The Physician or RN will complete.

A
  1. All pathology/cytology reports
  2. Operative reports
  3. All x-ray and scan reports pertaining to the patient’s diagnosis of cancer
  4. All consultation reports

How does the NUC arrange transport?

A

Arrangements need to be made with BC Ambulance, Transport Service or a family member.

A

BCCA appointment card.
Transcribe the next appointment on the Kardex and book future appointments prn, then attach the card to the inside of patients chart.

  15. if going to a clinic/doctors office will the stretcher fit in the waiting room
