A

HG T1
Any recurrent HG Ta
HG Ta > 3cm (or multifocal)
Any CIS
Any BCG failure in HG patient
Any variant histology
Any LVI
Any HG prostatic urethral involvement

CIS = carcinoma in situ; LVI = lymphovascular invasion
