83 YO male w/ bloody diarrhea develops renal failure. Has ischemic colitis. As diarrhea improves, urine output decreases. Peripheral smear shows red cell fragments. Stool culture is negative for a sorbitol-negative E. coli and stool sample is positive for Shiga toxin.
