What are the 2 types of meds used for AD?
- cholinesterase inhibitors
- N-methyl-D-aspartate (NMDA) receptor agonists
What are the 2 drugs that are cholinesterase inhibitors?
- Donepezil (aricept)
- Rivastigmine (Exelon)
What drugs is the NMDA receptor agonist?
Memantine (Namenda)
What drugs can be used for depression in pt’s with AD?
SSRI’s:
- Fluoxetine (Prozac)
- Mirtazapine (Remeron)
What drugs can be used for behavior problems in pt’s with AD?
Antipsychotics: -Aripiprazole (Abilify) -Olanzapine (Zyprexa) Benzodiazapines -Lorazepam
What does cholinesterase inhibitors do and and stage of AD does it treat?
- prevents breakdown of acetylcholine, increasing bioavailability
- may help to slow progression of disease
- mild to moderate disease
What are the side effects of cholinesterase inhibitors?
bradycardia wheezing diaphoresis miosis diarrhea salivation GI: n/v, dyspepsia, diarrhea dizziness headache
What are the drug interactions causing reduced therapeutic effects for cholinesterase inhibitors?
- anticholinergic agents
- 1st gen antihistamines
- tricyclic antidepressants
- conventional antipsychotics
What level of AD is donepezil (aricept) indicated for?
mild, moderate, or severe
This causes reversible inhibition of cholinesterase.

How often are the doses increased for donepezil (aricept) to decrease AE?
Q 1-3 months
What is one big difference between donepezil (aricept) and rivastigmine (exelon)?
- donepezil (aricept): causes reversible inhibition of cholinesterase.
- rivastigmine (exelon): irreversible inhibition of cholinesterase
What are the side effects of rivastigmine (exelon)?
- N/V, diarrhea, abd pain, anorexia, significant weight loss
- intensify symptoms in pt’s with peptic ulcer disease, bradycardia, sick sinus syndrome, fall related fractures
What is the NMDA receptor agonist Memantine (Namenda) indicated for?
- moderate to sever AD
- better tolerated than cholinesterase inhibitors
What are the side effects and drug interactions for the NMDA receptor agonist Memantine (Namenda)?
- dizziness, headache, confusion, constipation
- amentadine, ketamine, sodium bicarbonate
