No fault errors

Missed dx due to misleading information and it was nobody’s fault
System errors
Reflects health care system flaws
Examples include policies that are inadequate, poor coordination of care, inadequate training or supervision, poor communication, and a hosts of other related factors such as excessive workload, fatigue, distractions and stress
Cognitive errors
Results from insufficient knowledge or data gathering, inaccurate clinical reasoning or faulty verification.
Ex: jumping to conclusions, flawed perceptions, faulty logic, using biased heuristics and making final diagnosis too early.
