What to do for initial workup?
A
History, Physical Exam, IPSS, Urinalysis (UA)
Optional: uroflow, PVR, pressure-flow studies (UDS)

What are the treatments for BPH/LUTS?
A
- Behavioral modification
- Medical therapy
- Surgical therapy (refer to urology if PCP managing up to this point)

Follow up after initial workup/treatment?
A
Reevaluate 4-12 weeks after initiation of treatment to assess response. Use IPSS.
May use PVR, uroflow at this point.

What do you do for patient failing or not tolerating initial medical therapy?
A
Change medical therapy or move on to surgical intervention
Trial of alpha blocker or PDE5i for 4 weeks, or 5-ARI for 6-12 months is reasonable to assess for efficacy.
