Assessment of the fetus Flashcards
how is the fetus assessed for growth? two methods
serial sonograms
manually for the first half of the pregnancy (size of uterus)
what are five tests to assess fetal development?
kick counts non-stress test contraction stress test biophysical profile fetal acoustic stimulation
describe kick count test
about 10 kicks in 2 hours is considered normal
describe the non-stress test
HR increase of 15 BPM 2x in the course of 20 minutes
desrcibe the contraction stress test
a contraction is initiated via either oxytocin or breast stimulation and the fetal heart rate is monitored
late=bad
describe the biophysical profile test
fetal heart rate reactivity fetal breathing fetal movements fetal tone amniotic fluid volume normal = 2 points, abnormal = 0;
fetal acoustic stimulation
increase in HR with sound (supplied by artificial layrnx)
another round of indications for critical antepartum care
HTN, DM, previous stillbirth, post date (42 weeks)
what are five ways in which fetal maturation is evaluated?
amniotic fluid L/S ratio (lecithin/sphingomyelin) phosphatidylglycerol foam stability index fetal lung maturity ultrasound
describe the amniotic fluid L/S ratio
> 2 low likelihood of RDS
describe PDG test
> 3% = 1% risk of RDS
describe the foam stability index
surfactant tests, foam should last more than 47 seconds
describe fetal lung maturity
measures Surfactant to albumin ratio via flurescent polarization
US (two findings that decrease chance for RDS)
BPD (biparietal diameter) > 9.3 Femur length (FL) > 7.4
