to obtain medical-voice (case) hx; to perform instrumental assessment; to determine severity and stimulability of voice px; to explain findings to the patient; to set a treatment plan based on patients need and adherence; to answer questions and build rapport with the patient
what is NOT a goal of voice evaluation?
NOT to make a medical diagnosis - the ENT makes the medical diagnosis
components of medical hx (6)
prior surgeries; current medical condition / disease; physical disabilities; allergies; current medication(s); any others potentially affecting the voice
components of vocal hx (7)
prior voice px or tx; prior vocal training in speech or singing; onset and duration of voice px; daily vocal demands (kinds of usage); sensory symptoms (pain); coping strategies; vocal hygiene
how do you assess breathing? (3)
observe breathing pattern during spontaneous speech and at rest; place one hand on the abdominal wall and the other on the lower back - then place hands on the lateral ribs; repeat the second step depending on vocation (singers, voice actors, etc.)
what kinds of breathing are there? (4)
clavicular, thoracic, abdominal, variable
what else do you look for / observe when assessing breathing? (various)
abdominal wall excursion, shoulder raising, neck tension; small inadequate breaths; pushing voice beyond breath support; poor coordination between respiration and phonation
how do you assess for laryngeal muscle tension (what is assessed)?
laryngeal muscle tension (laryngeal palpation); jaw tension (palpate masseter); meck and shoulder tension (check ROM and palpate muscles)
What do we assess and look for (laryngeal muscle tension)? (5)
laryngeal height; thyrohyoid space; lateralization of the larynx; thyrohyoid and suprahyoid muscles; neck tension
