What are the objectives and components of ASK?
-Insight on clients goals and interests
-Explain CSEP options and confirm scope of assessments

What are the counselling tools for the FIRST consultation?
-Be sensitive, authentic, inclusive and professional
-Use strategic and open-ended questions
-Practice active listening
-Be observant and let the client do the talking
What do discovery questions do?
Discern a clients…
-Motivation
-Current activity levels
-History
-Knowledge and beliefs
-SOC
-Desire to undertake an assessment
What can a client not have 2 hrs before assessment?
No caffeine, heavy meal or nicotine
What can a client not have 6 hrs before assessment
No alcohol or strenuous exercise
What is the informed consent form?
-Required element for CPT
-Describes nature of assessment and outlines clients responsibilities
-Not a waiver
-Clients under 18 must fill out youth informed consent with a guardian
-Consents must be witnessed at the time of signing
What is the GAQ?
-Required element of CPT
-Must be witnessed by a 3rd party
-Clients answers determine whether they fall within CPT scope of practice or requires clearance from physician before partaking in physical activity
-Valid for 12 months unless health c=of client changes
What is the GAQ Reference Doc?
-If client says “YES” to anything on page one of GAQ refer them to this doc
-Helps CPT determine whether they need a healthcare practitioners approval before training
-Helps client make an informed decision about exercise safety
What are some reasons to refer out after initial consultation with client?
-Client is unable to exercise independently
-Client is at a higher risk for an adverse event
-Client wishes to train near maximal effort for aerobic or resistance training
-Client has HR and/or BP above screening requirements
What do you do if you need to refer to a physician (pre-screening)?
-Use Physician PA readiness clearance form
-Client must take both clearance form (to be completed and signed by physician) and GAQ to physician
