Why do we use the ASIA scale
It is a standardized way to define SCI and we can use it to help determine our patient prognosis
What is a full sensory screen
Sensory testing based on 28 dermatomes
Light touch and Sharp/dull touch
What is the grading for the full sensory screen
3 point scale (0,1,2)
2 - intact
1 - impaired (partial or altered inc. hyperaesthesia)
0 - absent
How many key myotomes is motor based on
10
5 upper
5 lower
plus a sacral exam: voluntary anal contraction
The sacral exam: voluntary anal contraction is graded by
Present or not present
What does MLI stand for
Motor level of injury
How is motor graded
6 point scale (0-5) 0 = no active movement 1 = muscle contraction 2 = movement thru ROM w/o gravity 3 = movement thru ROM against gravity 4 = movement against some resistance 5 = movement against full resistance
What is MIS
Motor Index Score (MIS) = total 100 pts
How is deep anal sensation recorded
Deep anal sensation recorded
“present vs absent”
May be the only evidence of a clinically incomplete SCI
Recommended method of testing is by digital examination The patient is asked to describe any sensory awareness, including feelings of touch and/or pressure

What is the grading system for the ASIA scale
A - E
What is an ASIA A
Complete: No motor or sensory function is preserved in the sacral segments S4-S5
(NOOOON sign)
What is an ASIA B
Incomplete: Sensory but not motor function is preserved below the neurological level and includes the sacral segments S4-S5 AND no motor function is preserved more than three levels below
What is an ASIA C
Incomplete: Motor function is preserved below the neurological level, and more than half of key muscles below the neurological level have a muscle grade less than 3
What is an ASIA D
Incomplete: Motor function is preserved below the neurological level, and at least half of key muscles below the neurological level have a muscle grade of 3 or more
