ASA I
Normal healthy patient
Non-smoker and no or minimal alcohol use
No acute or chronic disease
Normal BMI percentile
ASA II
Patient w/ mild systemic disease
No substantive functional limitations
Current smoker, social alcohol drinker, pregnancy, obesity BMI 30-40kg/m^2, well-controlled diabetes or HTN, mild lung disease
ASA III
Patient w/ severe systemic disease Substantive functional limitations 1+ moderate to severe diseases Poorly controlled diabetes or HTN Morbid obesity BMI >40
ASA IV
Patient w/ severe systemic disease that constantly threatens life
Recent < 3mos MI, CVA, TIA, or CAD, ongoing cardiac ischemia, severe valve dysfunction, shock, sepsis, DIC, ARDS, ESRD
ASA V
Moribound patient not expected to survive w/o the operation
Ruptured abdominal/thoracic aneurysm, massive trauma, intracranial bleed, ischemic bowel w/ significant cardiac pathology, multiple organ/system dysfunction
ASA VI
Declared brain-dead patient whose organs are being removed for donor (procurement) purposes
Pediatric ASA II
Asymptomatic congenital cardiac disease, well-controlled dysrhythmias, asthma w/o exacerbation, well-controlled epilepsy, non-insulin dependent diabetes mellitus, abnormal BMI percentile, mild/moderate OSA, oncologic state in remission, autism w/ mild limitations
Pediatric ASA III
Uncorrected stable congenital cardiac abnormality, asthma w/ exacerbation, poorly controlled epilepsy, insulin dependent diabetes, organ transplant history, premature infant, long-term parenteral nutrition
Pediatric ASA IV
Symptomatic congenital cardiac abnormality, congestive heart failure, acute HIE, ventilator dependence, severe respiratory distress, advanced oncologic state
Pediatric ASA V
ECMO, respiratory failure or arrest, malignant HTN, decompensated congestive heart failure, hepatic encephalopathy, ischemic bowel, MODS
Obstetric ASA II
Normal pregnancy well-controlled gestational HTN, preeclampsia w/o severe features, diet-controlled gestational diabetes
Obstetric ASA III
PEC w/ severe features, gestational diabetes complications, thrombophilia disease
Obstetric ASA IV
HELLP, peripartum cardiomyopathy EF < 40, uncorrected/decompensated heart disease
Obstetric ASA V
Uterine rupture
