A

Septica: Infeccion aguda

Reactiva: comienza 1-4 semanas depues de una infeccion en tracto gastrointestinal o genitourinario

A

Epidemiologia:

A

H:M 9:1 con antecedente urinario
H:M 1:1 con antecedente gastrointestinal

Mas del 80% tiene el fenotipo HLA-B27

Uretritis o cervicitis + Conjuntivitis o uveitis + Oligoartritis

Tratamiento incialy de 1ra eleccion:

A

AINEs

AINEs:

A

COX 1: No selectivos (producen sangrado gastrointestinal, no aumenta riesgo cardiovascular).
COX 2: Selectivos. Inhiben prostaglandinas a nivel cardiaco, por eso si más propenso a IAM = aumenta riesgo cardiovascular.

Si no responde con AINE: sustituye por esteroides temporales (prednisona)
Si no mejora (1 mes): ver crónica.

Tratamiento de Artritis reactiva cronica:

A

Sintomático: AINE o esteroide y
Sostén: añade sulfasalazina o Metotrexato

Si no responde: sustituir todo por un anti-TNF alfa

Anti TNF-alfa: Adalimumab, infliximab, Etarnercept
