Arthritis Flashcards
Why are post menopausal women more at risk?
Because oestrogen prevents osteoclasts, so when oestrogen stops, osteoclasts are no longer inhibited.
How quickly should Rheumatoid Arthritis be treated after symptons start?
3 Months
What does DMARDs stand for, and what are some examples
Disease Modifying Anti-Rheumatoid Drugs
- Sulfasalazine,
- Pencillamine,
- Gold Compounds,
- Anti- Malarials
What suppressant drugs are used to treat rheumatoid arthritis?
- Ciclosporin
- Azathiopine
- Methotrexate
- Leflundomidi
- Cyclophosphamide
What is Methotrexate used for
It’s an immunosupressant used for rheumatoid arthritis
What group do these arthritis drugs fall into:
Melocoxicam
Rofecoxib
Celecoxib
Etoricoxib
Selective Cox 2 inhibitors.
They prevent the conversion of aracadonic acid into prostaglandins, without inhibiting Cox 1 (useful in protecting GI mucose ext…)
What drugs can /are often combined with Methotrexate (the immune-suppressant)
Anticytokine Drugs
such as.... Adalimumam Entenercept Infliximab Rituximab Natalizumab
What is the drug ‘Natalizumab’
It’s a Anti-cytokine drug. As cytokines are ‘Bad/MAB’ for arthritis.
V Expensive so not used a lot and only given IV or SC. Not everyone responds
What drugs CANNOT be given with Gold Complexes
Penicillamine
A product of penicillin. Although a member of the Disease Modifying Anti Rheumatoid Drugs (DMARDs) they cannot be mixed.
What NSAIDS can be used for treating the symptons of Arthritis
Ibuprofen
Aspirin
Meloxiacam
Celecoxib
What is a drug with potential benefit for Osteoarthritis but is NOT reccomended by NICE?
Glucosamine Sulfate
As it has possible long term side effects
Whats:
MISOPROSTOL and how does it work?
It’s a synthetic prostaglandin which inhibits gastic acid secretion and may stimulate mucous production .
It’s given along side NSAIDS as it protects GI lining and protects against ulcers. DO NOT GIVE IN PREGNANCY- CAN INDUCE ABORTION
What Immunosupressant drug was first developed in Fungus?
CICLOSPORIN
Given orally but poorly absorbed. Used for transplant patients as has a high potency.
Why are steroids used for arthritis treatment.
E.G. Prednisolone, dexamethasone, fludrocortisone
Because they are anti-inflammatory (osteoarthritis) by blocking the gene transcription and synthesis of inflammatory proteins. And they are immunosupressant (rheumatoid)
