Arrythmias Flashcards
adenosine mechanism?
slows conduction by increasing K influx and decreasing Ca infulux
mechanism of BB?
slow conduction by blocking sympathetic input
mechanism of CCB?
reducing Ca influx thereby reducing the upstroke of the action potentional
new onset afib in ICU consequences?
inc mortality and inc LOS in ICU, inc risk of death after DC
first degree AV block?
long PR
second degree mobitz type 1?
lengthening PR until dropped beat
second degree mobitz type II?
constant PR with random dropped beat
RF for mobitz II?
age related fibrosis, AVN blockade, MI
tx for mobitz II?
PPM
what worsens prognosis in a CHB?
if its distal to AVN
MOA of WPW?
rapid accessory pathway bypassing the AVN
findings of WPW?
short PR, delta wave, can develop illegularly irregular wide QRS
tx for WPW?
procainamide, flecanide, sotalol, amio, ibutilide, cardioversion
AVNRT finding?
narrow QRS regular, like SVT. different morphology P wave
how long to give AC for post afib cardioversion?
4 weeks despite CHADSVASC
difference from dabigatran and rivaroxaban when comapred to warfarin?
non inferior and less bleeding events
congenital Long QT syndrome with exercise as trigger, what is the mutation in?
outward potassium channel
congenital long QT with rest or sleep?
sodium channel mutation
treatment of sick sinus syndrome?
dual chamber pacing
type of pacing done for heart failure with QRS widening, reduced EF?
biventruclar pacing
only arrythmia in which there is irregularly irregular wide and narrow complex tachycardia?
WPW
why no BB in WPW?
if you slow the atrial rate it will pass through the faster conducting accessory pathway instead of the AV node
MOA of procainamide in WPW?
increases the refractory period and decreases conduction through the accessory pathway
TOC in WPW?
procainamide and synchronized cardioversion
tx for brugada?
ICD and also quinidine if refractory
medication contraindicated in Brugada?
flecanide, procainamide, sodium channel blockade can precipitate brugada
who would get dual chamber pacing versus biventricular pacing?
DC is for heart block but biventricular is for patient with low EF
medical tx for torsades de pointes?
Mag (first line)
isoproterenol
lidocaine
overdrive pacing
how does propofol cause bradycardia?
vasovagal response
intubation meds that can cause bradycardia?
fentanyl, succinylcholine, propofol
electrolyte disturbances that cause prolonged QT?
hypomagnasemia, hypoclacemia, hypokalemia
hypothermia and SAH can predispose you to what arrythmia?
prolonged QT
anticoagulation post cardioversion despite CHADS2VASC?
4 weeks with warfarin, direct rhombin, Xa blockers
who with AF is CI to get DOAC AC?
moderate to severe MS or mechanical mitral
MR not a CI
dosing for eliquis in afib with normal kidneys?
5 bid
kidneys 2.5 bid or old >80 yo
other antiarrythmics for WPW?
procainamide, flecanide, sotalol, amiodarone
magnet over AICD does what?
prevents detection of tachycarrythmias, but pacing retained
magnet over pacemaker?
goes to asynchronous pacing at 60 bpm
